Patient

Continuity of Care and Patient Satisfaction Program

Post-discharge call center seeks patient satisfaction feedback and assesses their inpatient experience. Supports continuity of care by encouraging patients to follow through with their physician and, as needed, staff calls patients' physicians to ensure continuity of care. Engenders good will for the hospital while reducing risk.

Order Sets Program

Oversees order set development using evidence-based protocols reviewed and updated at appropriate intervals. Increases admitting efficiency, ultimately improving patient outcomes through reduction in errors, omissions, and unnecessary tests. Improves quality outcomes and core measure compliance.
