Hospitalist
A hospitalist is a physician whose practice is dedicated entirely to the care of hospitalized patients. The hospitalist is board eligible or board certified in internal medicine or family medicine. Many hospitalist physicians also have sub-specialty training and addition board certifications. Sound Physicians’ hospitalists are experts in clinical process excellence and delivering high-quality, compassionate care. Every Sound Physicians’ hospitalist has the opportunity to advance within the organization through leadership and partnership tracks called Path to Partnership™ and Path to Leadership™.
Hospitalist RN
The Hospitalist RN is a key member of the hospitalist team focusing on managing the SoundImpact™ initiatives, including our innovative High-Impact Diagnoses Initiative. This initiative is making significant improvements in quality, satisfaction and efficiency of patients within high-impact diagnoses managed at the hospital site. The Hospitalist RNs are a pivotal in achieving quality outcomes in diagnoses where volume, quality and cost overlap.
Roving Hospitalist
The Roving Hospitalist is an internal and superior version of locum tenens for the physician who enjoys a variety of assignments that feature a flexible schedule and new adventures in different geographic areas with Sound Physicians’ hospital partner sites. As a Roving Hospitalist you may also consider this as a career path to a permanent position down the road, enabling you to have a permanent “hospital home” when the timing and chemistry are right.
SoundConnect™ workflow and informatics platform
SoundConnect is a suite of proprietary web-based software applications designed by Sound Physicians’ hospitalists with our in-house software development team that support hospitalists in their daily activities. SoundConnect includes such applications as written sign-out, charge capture, daily census and AutoFAX communications to primary care physicians. SoundConnect also seamlessly integrates with SoundImpact™ initiatives. The SoundConnect platform serves as a data engine, collecting and analyzing data generated by Sound Physicians’ core measures and high-impact diagnoses initiative models.
SoundImpact™ Initiatives
SoundImpact includes several initiatives designed to reduce variance and improve the quality of care. Each model is focused on the intersection of quality, cost and efficiency of clinical workflow. SoundImpact includes Best Practices Initiative, Core Measures Initiative, High-Impact Diagnoses Initiative, Readmissions Management Initiative and Patient Satisfaction Initiative.
SoundInside™ management services
Sound Physicians invests deeply in our infrastructure to supports our hospital partners. Our internal resources include physician recruiting, training, analysis and reporting, billing, accounting, clinical operations, marketing and human resources.
SoundInstitute™ learning platform
Sound Physicians’ internal learning center funded and staffed by Sound Physicians. The Institute is dedicated to continuing education and professional growth. The Institute provides cross team networking and strives to implement best practices and evidence based care throughout Sound Physicians’ hospitalist teams. Based on the regional operations team’s monthly business review, specific training is applied to drive results for patients, our hospital partners and our physicians.
SoundTransitions™ continuum of care
Sound Physicians coordinates post-discharge services through SoundTransitions. Our in-house patient call center follows up with patients post-discharge. We develop strong relationships with primary care physician in the local areas we serve with a formal PCP agreement process. Our post-acute care service model is designed to provide high-presence in a range of post-acute care settings. We conduct regular satisfaction surveys with patients, primary care and specialist physicians, hospital administration and nursing staff, enabling us to have a pulse on how we are performing, by physician and facility.
