Every patient’s hospitalization begins with a care transition plan. As patients are discharged, their primary care physician or post-acute care facility receives pertinent information about the hospitalization and care plan. Our in-house SoundTransitions™ Patient Call Center contacts patients to ensure they are following the prescribed care plan. During the call each patient completes a survey regarding their hospitalization experience. With this data we are able to flag the risk of readmission, assess patient satisfaction by physician and identify specific opportunities for improvement for each hospitalist.
