Our foundation of strong communication with community primary care providers and specialists about our services and their patients strengthens the relationship between clinic-based physicians and the hospitals we serve.
Sound Physicians builds stronger community ties to its hospital partners by serving as a liaison to community physicians and specialists. Our hospitalist teams keep primary care providers informed of their patients’ progress during their acute care stay and at discharge. Our call center teams contact patients within four days of discharge, to make sure follow-up care with their primary care provider or specialist is scheduled and prescriptions are filled.
Ultimately, this communication and physician outreach helps us improve patient satisfaction, improve primary care provider and specialist confidence
in our services and reduce unnecessary healthcare
expenditures due to avoidable readmissions.
